Once you fund the trust you want to open any new accounts always in the name of the trust.
It's sort of like when you got married and you change your name.
From there on out you use the new name on the opening of any new accounts. Once you've created the trust, once you funded it, if you create any new account going into the future you're going to open it in your name as trustee of your trust.
