For your Advanced Directive & Durable Power of Attorney for Health Care, you are able to designate an agent(s) that will determine whether you’ve become incapacitated to the extent that you can no longer carry out your usual business affairs. You can choose anyone you trust to determine your incompetence, but we recommend the person you choose be at least 25 years old.

An agent is someone whom you appoint to act for you as if they were you.
To edit your selected incompetence decision maker(s):
1. Click on 'Your Info' at the top left of the program, then scroll down to select
'Health Care Decision Maker - Make Changes'.
2. Click on Incompetence Decision Maker - Make Changes
3. Here you are able to edit your selected agent(s) (Determinations of Incompetence)
